How do I book my virtual appointment?

There are multiple ways:

New Patients

  • Directly through our website: Just click on the appointment button and follow the prompts to book your own appointment at your convenience. It is really that simple!
  • Jump on the Healow website: healow.com.  From there you can:
  • Call our office to schedule.

Established patients can

  • Access your Patient Portal for a more efficient process.
  • Download the free Healow app from the Apple and Google stores for a seamless mobile experience. Practice code is CAFHDD.

Forms & Questionnaires

How do I complete my intake forms online?

Options Include:

  • Check in and enter the necessary details.
  • You may also go to your Patient Portal and click the Questionnaires tab.

Please complete your forms and questionnaires before your visit.

What should I expect during my consultation?

Your provider will review your history, discuss your symptoms, perform a virtual examination and review any studies you have had (medical imaging or labs). From there, a personalized care plan will be tailored to your specific needs.

Ways to access the Patient Portal?

  • Login credentials will be provided to you via email after you book your visit.
  • Click the Patient Portal Link on our website to go to our Healthcare Support page to access your portal and multiple other resources.
  • You can also Enroll online to create your Portal account.

 

Healow app:

  • healow app is a free, secure, and convenient way to manage what’s important and puts you in control of your health. It can be downloaded from the Apple or Google Play stores
What can I use my Patient Portal and Healow app for?
  • Self-booking appointments via online calendar.
  • Communicate directly with our providers.
  • Get reminders.

What should I expect during my consultation?

Your provider will review your history, discuss your symptoms, perform a virtual examination, and review any studies you have had (medical imaging or labs). From there, a personalized care plan will be tailored to your specific needs.

Uploading & Sharing Medical Imaging

Why is it important to have my medical images available to my provider?

Although most Orthopedic diagnoses and Wellness assessments can be made by obtaining a detailed medical history and performing a virtual physical examination, your provider is an expert in interpreting medical images and uses them to correlate these findings with your history and physical, in conjunction with the radiologist report.

How can share my medical imaging (X rays, MRIs and CTs etc.) with my provider?
  • We have established partnerships with various imaging facilities that have our specific imaging protocols and have committed to facilitating the care of our patients. This allows for the most efficient and accurate diagnosis of your specific Orthopedic condition. Using their physician portal, your clinician can view images online and access the official radiologist report.
  • We have also partnered with Mymedicalimages.com. They provide a fast and secure way for you to upload your images through a free, basic account. When you have had images done elsewhere, please upload them by clicking on the Upload button on our website.

Please inform your provider where you have had your images obtained or put a note in the comments section when you book your appointment.

What file types are accepted?

We accept DICOM, JPG, PDF, and TIFF formats.

Telehealth / Virtual Assesments

How do I prepare for my televisit?
  • It is recommended that you have a device with:
    • A working camera and microphone
    • Stable internet.
  • Suggested for your virtual Orthopedic exam:
    • A quiet, comfortable, private room with good lighting.
    • Exercise attire: Shorts, T-shirt or sports bra (females)
What if I lose connection during my visit?
  • You may reconnect using the original link or contact our office (message is best) if problems persist and we’ll manually send you another code.

Which browsers and devices work best?

Chrome and Firefox are best, although any browser will work.

How do I reschedule a telehealth visit?

Call our office or send a message through the portal to reschedule.

In-Person Visits

Where is the clinic located and where can I park?
What should I bring to my visit?

Bring ID, insurance info, medication list, and any relevant test results.

What if I need imaging or lab work done during the visit?

Because most of our in-person visits involve procedures, this is usually not necessary and typically are reviewed ahead of time.  However, we can help for arrange imaging or lab work at your visit (to be reviewed later or virtually).

What should I wear to my in-person appointment?
  • If you are being managed for an Orthopedic problem, it is best to wear shorts or T-shirt to allow the clinician access to your involved extremity.
  • If you are having a PNOE assessment, please see specific instructions below.
Can I bring a family member with me?
  • Yes, one companion may attend. All patients under the age of 18 require a parent or legal guardian to attend their child’s visit.
  • Please notify us ahead of time if you need assistance.

Understanding Your Care Plan

How do I follow the rehab or exercise plan given to me?

Your clinician will carefully review your custom care plan with you in person and make sure you have a good understanding of the concepts provided.  However, we also provide written instructions and select online videos are available in your portal. You are always welcome to contact us with any questions.

What do the metabolic results from my PNOE test mean?

Depending on the test you undertake, they provide insights into how your body utilizes energy and uses macronutrients/fuel (fats, carbohydrates and protein) as well as cardiovascular fitness (VO2 max) and performance capacity.

What’s the difference between physical therapy (PT) and personalized coaching?
  • In general, PT focuses on recovery, although there is a lot of overlap between the two disciplines. We are strong advocates of “pre-hab”, which is physical therapy provided before a major event, like surgery or a period of prolonged inactivity, to optimize your body’s functional capabilities and minimize potential decline that can occur.
  • Coaching generally includes exercise and metabolic health strategies as a means to optimize your health in conjunction with other injury and wellness preventative strategies.
How do I track my progress over time?
  • At your initial and then at each subsequent visit, you will be provided a detailed report of your health, nutrition, metabolic health and cardiovascular fitness.
  • Progress is tracked through repeat testing, clinical follow-ups, lab test and imaging comparisons.

Billing, Payments & Insurance

How do I know if you accept my insurance?
Can I use HSA or FSA for cash-pay services?

Yes, most cash services qualify for HSA/FSA payment.

How do I request a superbill for out-of-network reimbursement?
  • Contact us and we’ll email or portal-deliver the superbill with full codes.
  • If you are having an in-person visit, we will provide you with a superbill in person.
What is your payment policy for procedures like PRP, PNOE or uncovered braces?

Payment is due at the time of service. Receipts are provided for reimbursement.

Where can I access the necessary practice forms (e.g. Consent to treat and Healthcare questionnaires)?

These are available in your patient portal under the Questionnaire tab.

Can I use PNOE testing if I’m not an athlete or don’t exercise regularly?

Yes, it’s ideal for baseline wellness by evaluating your Resting Metabolic Rate (RMR).

Do you offer sports-specific assessments or training plans?

Yes, along with our expert athletic trainers and physical therapists, we can provide protocols tailored to your specific sport or athletic activity and level of participation.

How can I integrate coaching with my orthopedic care?

We offer integrated packages that combine visits, preventative plans, and remote support.

Referring Providers & Partnerships

How do I refer a patient for orthopedic evaluation or second opinion?
  • If you have are P2P user, please reach out to our clinic directly through this platform.
  • If you are not signed up for this free, convenient platform, you may sign up using this link.
  • You may always reach out to the clinic via email [email protected] or telephone and we will provide you a direct access line to our clinical staff.
What info do you need to provide a second opinion?

Please share patient history, images, and prior treatment notes.

How do you communicate with primary care or sports medicine providers?

We send reports and consults securely by fax or via P2P portal messaging.

Yes, custom services for injury prevention and rapid consults are available for business and athletic teams.  Please email us at [email protected] with some basic information and we will arrange a video conference to discuss your specific needs and packages available.
